Actually, What I'd like is a list of the KNOWN portals 
Per the rules, someone selecting "Path of the Stars" at character creation starts with one portal to which that character is attuned, most likely the one nearest their home. Perhaps it is a right of passage for val'Innares children to be taken to the nearest portal and told the process, just to see if they have the Talent.
My Character's home is in Almeric.
I'd just as soon NOT know the locations of the "hidden" or "forgotten" portals. The adventure is in the discovery of the new.

Hello Will,
If I remember correctly, the known portals in Almeric is just outside the town of Lucheck, as detailed in the first Classic Living Arcanis adventure, So Shall Ye Reap.

Do I simply list "Lucheck" as my home/strart Portal? Or do the portals have specific names?

You are from NEAR Luchek or Navanne. You can be from anywhere in the area.

No Portals do not have specific names, other than location identifiers, such as "the Portal near Lucheck", etc.
Cody is also correct is stating that there is another in Nevanne.
