Is there a template out there for creating our own custom threats?

The last chapter of the Bestiary goes over how to create your own Threats, step-by-step. Because of how the variations work together, putting it into a spreadsheet would be very difficult to format, and in the end could still end up producing some very unbalanced Threats.
Or are you asking for just the template to format the Threats correctly, which is in the adventure template?

Template for formatting correctly. I have need and desire to make crazy monsters of doom.
